ACL Tears in Young Athletes

Youth sports help children and teens stay active, build confidence, and develop lifelong healthy habits. However, they also increase the risk of sports injuries, including one of the most common and serious knee injuries in young athletes—an ACL tear.

The anterior cruciate ligament (ACL) is one of the primary ligaments that stabilizes the knee. An ACL injury can occur suddenly during sports that involve cutting, pivoting, jumping, or rapid changes in direction, such as soccer, basketball, football, volleyball, lacrosse, and softball. Recognizing the early signs of an ACL tear in young athletes and seeking prompt evaluation can help prevent additional damage and support a safe return to sports.

What Causes an ACL Tear?

Many people assume ACL tears only happen after a direct hit to the knee, but most injuries actually occur without contact.

Common causes of an ACL tear include:

  • Suddenly changing direction while running
  • Landing awkwardly after a jump
  • Pivoting on a planted foot
  • Stopping quickly while sprinting
  • Direct contact during football or other collision sports

Young athletes participating in year-round sports or high-intensity training may also have an increased risk of knee injuries.

Early Signs of an ACL Tear

An ACL tear doesn’t always look dramatic, and some athletes may even try to continue playing. Knowing the warning signs can help parents, coaches, and athletes recognize when it’s time to see an orthopedic sports medicine specialist.

A “Pop” in the Knee

Many athletes report hearing or feeling a popping sensation at the time of injury. While not every ACL tear causes this symptom, it is one of the most common signs.

Rapid Swelling

Significant knee swelling that develops within a few hours after an injury often indicates damage inside the knee joint.

Knee Instability

Does the knee feel like it’s “giving out” when walking, running, or changing direction? Instability is a hallmark symptom of an ACL injury because the ligament is no longer providing normal support.

Pain with Activity

Some athletes experience severe pain immediately after the injury, while others notice discomfort mainly when trying to run, pivot, or bear weight.

Limited Range of Motion

Difficulty fully bending or straightening the knee may signal swelling or structural damage that requires evaluation.
