Many schools require this exam before a student can participate in practices or competitions, but parents need to understand what a sports physical can and cannot identify.
A sports physical is designed to determine whether an athlete is generally healthy enough to participate in sports. However, it is not intended to diagnose orthopedic conditions, evaluate lingering joint or muscle pain, or identify underlying injuries that could affect performance or increase the risk of future problems. When an athlete experiences pain, recurring injuries, or movement limitations, an orthopedic evaluation by a sports medicine doctor provides a much more detailed assessment of the bones, joints, muscles, ligaments, and tendons.

What Is a Sports Physical?
A sports physical, also called a pre-participation physical examination (PPE), is designed to determine whether a student is healthy enough to participate in school sports and other athletic activities. Schools often require a sports physical before students can join teams for football, soccer, basketball, volleyball, baseball, softball, track, wrestling, and other sports.
During a sports physical, your healthcare provider will review your child’s overall health, including:
- Medical history
- Previous illnesses or surgeries
- Past sports injuries
- Blood pressure and heart health
- Lung function
- Vision screening
- Height and weight
- General musculoskeletal screening
The goal of a sports physical is to identify any medical conditions that could make sports participation unsafe. However, it is not intended to diagnose orthopedic injuries or chronic joint pain.
Sports Physical vs. Orthopedic Evaluation: What’s the Difference?
Although both appointments involve evaluating an athlete’s health, they serve very different purposes.
A sports physical is preventive. It determines whether an athlete can safely participate before the season begins.
An orthopedic evaluation is diagnostic. It identifies the cause of pain or injury and creates a treatment plan to help athletes recover safely.
If your child has persistent pain before the season starts, don’t assume a sports physical is enough. An orthopedic specialist can evaluate the injury before it becomes more serious.
Signs Your Student-Athlete Should See an Orthopedic Specialist
Many young athletes try to “play through the pain,” but continuing to compete with an untreated orthopedic injury can lead to longer recovery times and more significant problems.
Schedule an orthopedic evaluation if your child experiences:
- Knee pain during or after sports
- Shoulder pain while throwing or swimming
- Hip pain that limits movement
- Back pain during practice or competition
- Ankle sprains that continue to recur
- Foot pain while running
- Joint swelling
- Limited range of motion
- Pain lasting more than a few days
- Difficulty returning to normal athletic activities
