Charitable Donation & Sponsorship Application

Thank you for considering OSMC as a community partner for your event or organization.

Because we receive a high volume of requests each year, we have outlined the following guidelines to ensure your submission receives proper consideration.

Our Commitment

OSMC’s mission is to provide exceptional, patient-centered, physician-led, and coordinated care to the patients and communities we serve. We prioritize partnerships that support this mission and strengthen community well-being.

Guidelines for Consideration

Eligible organizations typically meet the following criteria:

1. Are a registered non-profit organization
2. Promote health, wellness, or community well-being across diverse age groups
3. Positively contribute to defining and supporting the community
4. Offer partnership opportunities between OSMC and the organization
5. Include an educational component that supports shared learning and strengthens the partnership between OSMC and the organization
6. Align with OSMC’s mission and strategic priorities

Sports League & Team Sponsorships

OSMC is committed to supporting sports programs of all ages and promoting safe, active lifestyles. Due to the high volume of requests each year:

• Sports Leagues & Teams (All Ages):
  Sponsorship opportunities are very limited and reviewed based on alignment with OSMC’s mission, community impact, and ability to promote health and safety.
• OSMC Team Sponsorships:
  We are not accepting additional team sponsorship requests for the 2026 calendar year.

A separate application process for 2027 sports league and OSMC Team sponsorships will open in Fall 2026.

All sponsorship requests are evaluated annually using the same review cycle described in the guidelines.

Review & Approval Process

1. All organizations must complete the official application form to be considered. Additional materials may be submitted—please indicate this on your form.
2. A maximum of one request per organization may be approved per calendar year.
3. Donations are not automatically renewed. Each request is evaluated independently, regardless of prior participation.
4. All charitable and sponsorship requests are reviewed annually in the fall for consideration in the following calendar year.
5. Beginning in 2026, advance communication will be sent prior to the application process opening.

While all submissions are reviewed, please understand that we are unable to fulfill every request.

Each proposal is reconsidered annually, whether or not it was approved previously.
