OSMC NOW HAS MOBILE CHECK-IN
We’re excited to now offer mobile check-in. Please look for an email or text message from "Phreesia" a few days before your appointment. It will allow you to register for your appointment from home and at your convenience. It will also allow you to complete valuable health questions and information before you visit an OSMC provider.
If you don’t get a chance to complete the online paperwork, please arrive at least 15 minutes before your appointment to allow time to complete it in-office.
